Hub Pilot Projects Take Flight

With support from the Humboldt Community Health Trust, the North Coast Health Improvement and Information Network (NCHIIN) and Humboldt IPA, are piloting “hub” services to support locally contracted Enhanced Case Management, Community Support Providers, and other interested parties.

Early pilot services include using the North Coast Care Connect Community Information Exchange for shared data and case management functionality, along with hub-based billing services. This hub model aims to connect individuals and families with the care they need while streamlining administrative services for providers.
