Exclusive Early Entry and Nomination Process Opens for IL HCP Second Cohort | |

To address the urgent need for more house calls in Illinois, HCCI launched the Illinois House Call Project (ILHCP) in 2022 with highly qualified Illinois medical practices. This first cohort is well on its way to exceeding its three-year goal.
HCCI is now offering Illinois practices a period to be considered for exclusive access to the application process for ILHCP Cohort 2 (launching in early 2024), or nominate another practice for consideration. Like the first cohort, the second will include independent entities, those affiliated with hospital systems, and specialty groups ─ and will also include hospice and palliative care programs.
Each practice will designate up to three “Home Centered Care Champions” to lead their work. HCCI will provide the Champions with expertise, guidance, and comprehensive resources, including education, consulting, tools, market analysis, marketing/PR training, and the opportunity to receive capacity-building funding.
To be considered for exclusive access to the ILHCP Cohort 2 application process, or nominate another practice for consideration, complete this form by January 12, 2024.

The American Academy of Home Care Medicine (AAHCM) has relaunched its national practice directory under the “House Call Finder” name. According to AAHCM, the House Call Finder is a “comprehensive and accessible directory for home-based medical care across the nation. The key features of the directory are:
• Comprehensive Listings: Our directory encompasses a wide array of practices that specialize in home-based medical care, providing patients with varied options to suit their needs.
• Search Flexibility: Users can efficiently search for practices based on ZIP code or keywords, making it convenient to find nearby home-based medical care services.
• Detailed Information: Listings include details such as hospital affiliations, in-home services offered, and availability for urgent visits, enabling informed decision-making for patients and their families.”
AAHCM asks that participating practices help them provide the most up-to-date information in the House Call Finder by reviewing their listing and updating their information as needed. Practices wanting to be added to the directory can also use the form.
