How to Upload Your Vaccine Cards
- View the step-by-step guide on how to upload your vaccine card into the OptimumHQ.

Visit “My Check-Ins” in the OptimumHQ dashboard under Vaccine Proof to verify your vaccine card has been added. Look for the same file name you used to save your vaccination card.

Vaccine cards will be verified in the state system. Any fraudulent or altered vaccine cards will result in student discipline and being dropped from any enrolled in-person or hybrid courses.
Alternatives: Take Classes Online
If you miss the January 3 deadline to submit proof of vaccination, take classes online. Open enrollment closes on January 9, 2022, if you need to switch or enroll in an online or remote course, be sure to do so before January 9, 2022. See our list of online classes in SchedulePlus.

Enroll in Classes that Start later in the Semester
If you need more time to meet the vaccination requirement for in-person and hybrid classes, consider enrolling in classes that start later. We offer classes that start in February, March, and April. Visit the Schedule of Classes via Schedule Plus to see your options for late-start classes.

To request a medical or religious exemption, please visit our website for submission forms.
- Submit your exemption request by January 2.

If you are approved for a medical or religious exemption, you will be required to submit weekly negative COVID-19 test results via OptimumHQ.
