Temporary Library Card Reminders
Before You Apply:

  • Please note: current C/W MARS library card holders are ineligible for temporary Quarantine Card (Q-cards). If you are unsure whether you already have a regular library card, please call the Southborough Library at 508-485-5031.

  • This is a temporary library card; it is designed to allow you to access digital Southborough Library e-resources as well as curbside pickup of materials for the duration of the COVID-19 pandemic. 

  • When the library reopens, temporary card users will need to verify their address in-person at 25 Main St. Southborough, MA 01772.

  • This service is intended for Southborough residents. Residents of other Massachusetts municipalities are encouraged to contact their local libraries for access. Requests outside Southborough will be considered on a case-by-case basis.

To Apply:



  • The information submitted on this form must be accurate and complete. Please make sure to provide your first and last name in the form.

  • In addition to confirming you are a MA resident, applicants must be 13 years or older. Children under the age of 13 can have a parent or guardian register. Children ages 13 and older should typically fill out the application form themselves.

  • Please notify the library immediately if your address or contact information changes or if your temporary card is lost.

  • This card is not to be used by other individuals other than they person who is applying. Each individual in a household should have their own personal library card.

  • Incomplete application forms can not be processed. The library is unable to issue temporary Q-Cards to patrons who do not respond to follow up application inquires.
Getting Your Temporary Q-Card:

  • Upon successful completion of this application form, temporary Q-Cards will be mailed directly to patrons at home.

  • Due to the amount of recent applications, the library is unable to send a confirmation e-mail. Please wait for your physical library card to arrive in the mail.
