Dear Har Zion Day Camp Families,
The first 2 forms are required for our Health and Human Services and State Private Education licenses.
Please print them out and have them filled in, signed and returned to The Camp office, 1500 Hagys Ford Road, Penn Valley, PA 19072, by June 1.
1. Child Health Assessment
Please note that no child will be admitted to camp without proof of immunizations. Immunizations received must be listed and signed by your physician.
2. Emergency Contact Form
When completing the Emergency Contact Form, the state requires that every line be completed. Please do not write "same as above" on any line. If a line is not applicable, please write "NA"
Please be sure to sign and date the bottom.
3. Parent Handbook
Please read over the handbook. It contains information you need for the camp.
