As you have likely heard, it is anticipated the FDA will approve the first vaccine for COVID-19 in early December. As part of the local vaccination planning efforts, the Chicago Department of Public Health has reached out to the Chicago Dental Society for support in its efforts. One of CDPH’s first steps is to understand the extent of actively practicing dentists in the Chicago city limits who may be interested in receiving a COVID-19 vaccine when it is available.
As planning and executing vaccinations is a huge undertaking, the CDPH is asking that dentists complete the survey below to gather information on practice locations and hours, staff numbers and contact information so the department’s planning team can coordinate vaccine quantities and logistics for interested dentistsYou are not making any commitments at this time.
Dentists as well as their staff fall into the category of healthcare providers who will be eligible to receive the first round of COVID-19 vaccination, however, priority will be given to healthcare professionals who are working directly with COVID-19 patients.
Please complete this short survey as soon as possible. Results will be tracked by CDPH and used in its planning efforts. Their staff may also reach out to you directly with more information after you complete the survey.
It is important to note that CDPH is coordinating the vaccination rollout only within the City of Chicago, so this survey is only being sent to dentists with active practices inside the city limits. Vaccination efforts for dentists outside city limits will be coordinated by county and state departments of public health.
