Reminder: Post OSHA 300-A Summary
This is a reminder that your OSHA 300-A Summary of Work-Related Injuries and Illnesses for calendar year 2018 is required to be posted in a conspicuous location or placed where employees frequent from February 1-April 30, 2019.

Click here for a copy of the OSHA Form 300 and Form 300-A Summary.  Below are instructions on how to complete the Form 300-A:

To complete the annual OSHA 300-A Summary, you must:
  • Total the columns on the OSHA 300 Log (if you had no recordable cases, enter zeros for each column total); and
  • Enter the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log.
  • If you are using an equivalent form other than the OSHA 300-A summary form, as permitted under ยง1904.6(b)(4), the summary you use must also include the employee access and employer penalty statements found on the OSHA 300-A Summary form.
NOTE: The only exemptions to completing and posting this form are if you had fewer than 10 employees at all times throughout the calendar year (if you had 10 or more employees at any time, even if it was a temporary hire, you must complete and post the OSHA 300-A Summary) or for establishments in certain low-hazard industries, such as Elementary and Secondary Schools.
