Online: Emergency Info & Student Release and Health Update:
Complete the Data Verification on the Source before Sept. 30.
Be sure we have your most up-to-date information and contacts.
High School FERPA: Complete and return if applicable.
Laptop Device Agreement Form: Complete and return.
Form 504-2: (Identify Students with Disabilities) – Return if applicable.
Housing Questionnaire: (McKinney-Vento Program) – If applicable.
Native Education ED 506: (New Students Only) – Return if applicable.
Apply for Free/Reduced Meals: Students may be eligible for reduced fees. Complete online or if paper, return to the Ballard HS Lunchroom.

JED Foundation Survey: Please read Introductory Letter;
If you would like for your student to participate in the survey, complete by Oct. 11. Provide consent using: Ballard JED Consent
Ballard Food Bank Weekend Food For Kids Info and Enrollment Form.
Nurse Letter and refer to the BHS School Nurse page.
Medication Authorization Forms must be completed for all (prescription & over the counter medications) on campus.
