Important Message from GCCCD
At the Grossmont-Cuyamaca Community College District (GCCCD), we are committed to providing students with a safe and healthy campus and classroom environment.
 
All students enrolled in on-campus classes at Grossmont or Cuyamaca College must submit proof of vaccination status for Fall 2021. Students failing to provide proper documentation or an acceptable request for exemption will be dropped from on-campus classes.

This email is a reminder to upload your vaccine documentation or request a medical or religious exemption by Monday, August 9, 2021.

SUBMITTING VACCINE DOCUMENTATION

Please submit your documentation by Monday, August 9, 2021, to attend on-campus classes or seek in-person services beginning Monday, August 16.

One of the following documents are acceptable:

1.   Scan/photo of the front of your COVID-19 Vaccination Record Card,
2.   Photo of your Digital COVID-19 Vaccine Record (QR Code) from the California Immunization Registry, or
3.   A medical document from your doctor.

All of the above must include the dates of your vaccination and the manufacturer brand (Pfizer, Moderna, Johnson & Johnson).
 
 

REQUESTING AN EXEMPTION

Exemptions will be considered for medical and religious reasons only. Students will need to complete the correct form with appropriate signatures and documentation no later than Monday, August 9, 2021, for exemption consideration by August 16, 2021.
 
The forms can be downloaded and once complete and signed, should be uploaded to Colleague/Self-Service.
