Look for your member ID card and welcome kit.

You should have already received your member ID card and welcome kit in the mail.


  • Keep your ID card in a convenient place so you can show it when you receive covered services.


  • Complete the Health Risk Assessment (HRA) form included with your welcome kit. This brief, confidential questionnaire helps us learn how we can serve you best. Be sure to mail it back to us to earn a $30 reward.

Get to know your benefits.

Your best resource for plan information is your Evidence of Coverage (EOC). It includes all the details about your coverage and costs, including copays and allowances.

Get to know your network.

We have an excellent network of primary care providers (PCPs), specialists, urgent care clinics, and more. When you see an in-network provider, you not only get care from the doctors who know you best—you get it at the best value, too.
