Dear Parents/Guardians,
All incoming freshmen and transfers are required to submit the following health records to the Main Office by September 11, 2026:
- Complete and up-to-date immunization records
- Health physical examination completed within the current year
If your student has a medical condition or health concern, appropriate documentation from the student’s healthcare provider must also be submitted to the Main Office, Attention: Mr. Matthew Cumber.
Students with asthma or a seizure disorder must have the following on file:
- A current Asthma Action Plan or Seizure Action Plan completed by the student’s physician
- Any medications needed to manage the condition
- Emergency medication required by the student must be supplied by the parent/guardian and kept in the School Nurse’s office, in accordance with school requirements.
Please ensure that all required health records and documentation are submitted directly to the Main Office, Attention: Mr. Matthew Cumber, by September 11, 2026.
Having these records on file is essential to ensure that we can properly respond to your student’s health and safety needs while at school.
