Dear Families,
I hope you are enjoying summer! While there still plenty of summer left, I am hoping to start some of the back-to-school preparations now. There are health forms on Blackbaud that need to be completed before school begins. Returning families have three forms for each student.
- Emergency Contacts must be reviewed and updated so we have current information.
- Health Records Requirements lists exactly what your student needs to attend school per NYS law.
- Submit Health Forms is optional but provides an encrypted place to submit health records if you do not want to email or fax them.
Please complete these as soon as possible because there are time constraints for vaccinations, and returning to school is already a busy time. I’m happy to address any questions or concerns, my contact info in below.
