Mercy Care would like you to join us in welcoming the newest addition to its SMI Network! Mercy Care is excited to announce Resilient Health’s 1st Street Clinic as the newest addition to the Mercy Care SMI Network.
The 1st Street Clinic is located at 1415 N. 1st St., Phoenix, AZ 85004 and reached telephonically at their toll-free main number, 877-779-2470. The new location will be an Integrated Health Home.
The 1st Street Clinic will offer:
- Primary care services
- Lab services
- Peer support
- Case Management
- MAT services
- Psychiatric evaluations
- Medication management
- First Episode Psychosis
- Counseling
- Various groups (including substance use)
The clinic also offers GMH services for adults and youth.
