Live Well at Home Grant

The Live Well at Home Grant Request for Proposals is now open

The purpose of this grant is to develop and/or provide services for older Minnesotans to live and age in the community of their choice.

DHS – Customized Living Quality Improvement Grant

The Minnesota Department of Human Services (DHS), through its Aging and Adult Services Division (AASD), seeks proposals from qualified respondents to improve the overall quality of services for people receiving customized living services.

Qualified respondents must have provided customized living services to Brain Injury (BI), Community Alternatives for Community Inclusion (CADI) or Elderly Waiver (EW) program participants in the past two calendar years (i.e., between Jan. 1, 2024, and Feb. 23, 2026). 

MBA Dementia Grant

Grant is due March 20, at 4:00 PM

The Minnesota Board on Aging administers a competitive grant program focusing on Alzheimer's disease and other dementias, as well as their impact on families, friends, and caregivers.

Emergency Planning Workbook for Caregivers

Caregivers sometimes worry about what would happen if they couldn’t provide care for a short time. The Emergency Planning Workbook on the Minnesota Caregiving (Trualta) platform helps families prepare by documenting important details such as routines, medications, and emergency contacts. This tool helps caregivers feel more confident and ensures their loved one continues to get safe, steady support.

Caregiver: A Love Story

Wednesday, May 27 at 11:00 AM PDT | $5.00 Per Person

About the film: When Bambi chooses to stop cancer treatment and spend her final days at home, her husband, Rick, becomes her full-time caregiver with little experience or support. At first, things go well, but once the family leaves, Rick faces exhausting, nonstop caregiving on his own. Caregiver: A Love Story shows how hard caregiving can be and encourages people to think about better ways to support caregivers.

Sibley County Expo

April 9, 2026

The Sibley County Senior Expo is held each year in April. This year, there are craft and vendor booths, as well as speaker topics on caregiver support, health care, eye health, and mental health. A noon meal and main entertainment are included as well.  Locations where tickets can be purchased are listed on the flyer, or they can be purchased at the door the day of the Senior Expo.

2025 Age-Friendly Regional Exchanges

Last November, the Minnesota River Area Agency on Aging, Bethesda, City of Willmar, and AARP Minnesota partnered with Age-Friendly Minnesota (AFMN) to co-host the Southwest Age-Friendly Exchange, one of six Regional Exchanges held around the state in fall 2025. Several attendees from the region and across sectors gathered to share about age-friendly work and explore opportunities ahead.
