The Health Council is conducting an updated review of high-risk health technology (HRHT) currently in use across Bermuda’s health system. This questionnaire has been developed to capture data HRHT to be used for assessments in accordance with the paragraph 4 of the Bermuda Health Council (High Risk Health Technology) Regulations 2024 (link). The submission deadline for this questionnaire has been extended, and we kindly ask that it be completed by 5:00pm on 18^th January 2026.

Instructions for Completing the Questionnaire

To ensure data accuracy and proper categorisation:

• In Section II. Health Technology Information of the questionnaire, the box titled Health Technology 1 is to be completed for one (1) piece of health technology. 
• If you wish to provide information for additional technology, click the “Add Another Technology” button at the bottom of the page to enter each additional technology separately.
