Thank you for your participation in the 2024 Clinical Integration (CI) Program. We have summarized your performance in the 2024 CI Program and have placed a 2024 CI Index Report with your scores in your practice’s OneDrive account labeled as {Firstinitial_Lastname_Last four digits of your NPI}_ PRELIMINARY _ CI _ 2024 _ ADULT_SCORE_{Date}. We have also placed supporting patient documentation in your OneDrive account. 

 

In addition to your practice receiving 2024 quarterly care coordination payments based on patient attribution, providers who earn a CI Index score of 25 or above and who have met their 2024 CI Program participation requirements are eligible for a 2024 performance-based incentive payment. 


We rely almost exclusively on payer claims data to calculate quality performance, which at times may not provide a complete view of quality performance. We invite you to supplement this claims data with clinical data from your EMR to give us a more complete picture of the quality of care your practice is delivering (see “Supplemental File Submission Process” below). 


How are CI Index incentive payments determined? 

Incentive payments are distributed based on a provider’s CI Index score, and are also dependent on network performance, the amount of available funds, and the physician’s performance relative to his/her peers within the Clinically Integrated Network (CIN). You can review your CI Index score in your practice’s OneDrive account as well as patient lists with documentation of measure non-compliance


Supplemental File Submission Process   

We understand that sometimes a patient has received a service that may not be captured in the payer claims data we use for our calculations. If you have evidence that a service was completed within the measurement period and is not accounted for, please contact yourPopulation Health Manager and/or Population Health Specialist immediately to discuss next steps regarding your Quality Performance Metric (QPM) score and/or Clinical Documentation Metric (CDM) score. They will advise you on how to submit supplemental files. All supporting documentation required for MSHP to review the data must be submitted by your practice by August 22, 2025.  

To expedite review of your scores, please view this document on how to name and upload your documentation to your practice’s One Drive account.

Notes:


  • Your CI Index ranges from 0-106 points and represents your level of clinical integration based on the following elements: 
  • Quality Performance Metrics (worth up to 40 points) 
  • Clinical Documentation Metrics (worth up to 30 points) 
  • Efficiency Performance Metrics (worth up to 30 points) 
  • In-Network Integration Metric (worth up to 6 bonus points)  
  • The supplemental file submission process is for numerator credit only. Patients cannot be removed from a measure’s denominator. 
  • See the above guide for a list of measures eligible for the supplemental file submission process review.  
  • Please submit any reports by the August 22, 2025 deadline, as we are unable to review reports after that deadline. This deadline is in place to ensure that we can process timely incentive payments for the network. 
  • If you submitted sufficient supplemental data to close care gaps for Empire Commercial patients and/or Empire Medicare Advantage patients in June 2025, as requested by MSHP, that data will be pre-populated in the patient list that is provided to you by MSHP during the supplemental file submission process. No further documentation is needed from you. Your practice will automatically receive Quality Performance Metric data credit that will be applied to your 2024 CI Index Score 
  • Following the supplemental file submission process, all providers will receive a final CI Index Report in their practice’s OneDrive folder.
