We'd like to take a moment to thank you all as we reopened in-person visitation this week. It has been a smooth process so far, and we appreciate everyone's respect for the protocols that are in place to keep everyone safe. Visitation Information
In preparation for your upcoming visit, please review the following:

Preparing For Your In-person Visit

  • Visitation is now available Monday - Friday, 11:00 a.m. – 7:00 p.m. Weekend appointments can be scheduled between 10 a.m. and 2 p.m. by calling 716-639-3311 ext. 2128 or by emailing [email protected]. Weekend appointments need to be made by 2 p.m. on Fridays.

  • You are not required to present a negative COVID-19 test for visitation. However, should you want to have a rapid COVID-19 test done onsite prior to your visit, you must schedule a time Monday - Friday to come in and receive your test. You can schedule the test by calling 716-639-3311 ext. 2210. Prior to your test, you will need to complete the COVID-19 testing form.

  • If your loved one is currently on a 14-day quarantine, they are not eligible for in-person visitation. However, virtual visits are still available.

What To Expect When You Arrive

  • Please use the entrance at 2700 N. Forest Rd.

  • All visitors will be screened for COVID-19. This will include a temperature check and you will be asked questions about potential exposure to the virus and recent travel. If you have signs or symptoms of COVID-19 and/or have had close contact with someone with COVID-19 infection in the prior 14 days (regardless of your vaccination status), you will not be allowed to have your in-person visit.

  • All visitors will be required to use hand sanitizer upon arrival and must wear a mask that covers your nose and mouth at all times during your visit at the facility.

  • You are only allowed to travel to and from your loved one's room. You are not allowed to travel anywhere else within the unit or the facility at any time.

  • Individuals must remain 6 feet apart throughout the visit.

  • For residents living in a semi-private room, visitors are not allowed to enter the living space of the roommate or the shared bathroom.

  • Up to two individuals will be permitted to visit a resident at any one time.

  • Individuals under 16 years old must be accompanied by an adult over 18.

These infection control principles will be adhered to at all times. Visitors who are unable to adhere to these core principles will not be permitted to visit and will be asked to leave.

Any visitor who enters the facility and at any time receives a positive COVID-19 test result will be required to notify the facility as soon as possible.

Per DOH guidelines, when a new case of COVID-19 is identified among residents and staff, as required, the facility will immediately begin outbreak testing and suspend all in-person visitation.

Thank you for following these infection control practices to ensure everyone's safety. We appreciate your cooperation and understanding.

Important Update
Previous DOH regulations regarding holiday guidance is no longer in effect. Should a resident leave the facility for a non-medically necessary appointment, they will no longer need to do the following:

  • Produce a negative COVID-19 test within 24 hours of their return.

  • Sign the attestation previously in place for residents and family members.

  • Be tested three times within their 14-day quarantine period. We will instead conduct a COVID-19 test the day the resident returns, and one more test on day 14 of their quarantine period.
