This peer group will focus on exploring and strengthening partnerships that support genuine collaboration and mutual respect.
Key concerns will include understanding and navigating power dynamics, finding alignment and reducing duplication, and partnering effectively to encourage thriving communities.
Awardees will share insights, strategies, and challenges in building more productive and balanced partnerships to improve maternal health outcomes in rural communities.

Expanding the Maternal Care Team: Rural Strategies for Improving Outcomes

Join us to hear from rural communities leading the way in transforming maternal care teams. Panelists will share their innovative approaches to expanding the maternal health workforce, including the integration of community health workers, doulas, midwives, and other essential clinical and non-clinical roles. Participants will also have the opportunity to engage in a discussion with the panelists and learn from their experiences.

Grant Closeout Information
Although the end of your project period may be extended if you are granted a no-cost extension (NCE), here is some grant closeout information. We recommend you begin preparing for closeout approximately 1-3 months prior to the end of your grant.
Please ensure that the contact information in the electronic handbook (EHB) is updated and accurate. If you have questions about how to update contact information, contact the EHB help desk.
What is grant closeout?
HRSA requires submission of information on progress, equipment (purchase and disposition), supplies and a final Federal Financial Report (totally liquidated) at the conclusion of grant funding.
Within 90 days after the end of the project period, complete these activities:
- Reconcile financial expenditures
- Liquidate all obligations incurred
- Submit final Financial Report to HRSA
- Submit Federal Financial Report to Payment Management System
- Submit Final performance/progress reports, as required by HRSA’s program award terms and conditions (see attachment for more information about reports)

- In addition, you will receive closeout guidance from the HRSA Closeout team during the end of your grant period.

Year 4 Performance Data Reporting
The Year 4 Performance Data Reporting submission is due in the HRSA Data Collection Platform on May 31, 2025, as noted in your Notice of Award. This due date applies to all awardees, even if you are granted an NCE.
Final Closeout Report
The Final Closeout Report is due in the electronic handbook (EHB) within 90 days of the project end date. If you do not request an NCE, the report is due on or before July 29, 2025. If you are granted an NCE, this report is due within 90 days of your new and approved project end date. A template for the Final Closeout Report will be shared when it is finalized and approved.

New Quality Program Leadership
Thank you to those of you who were able to join us March 20, 2025, for the Zoom call that introduced the new program coordinator and project officers who will lead and support the rural quality program. For those who could not join, Kristi Martinsen, a Senior Advisor, and former Hospital State Division Director in FORHP is in the Quality Program Coordinator role. Joining Mew Pongsiri, who still has her portfolio of quality grantees, we have Siobhan Dempsey, Lorraine Burton, Lawrencia Afagbedzi, and Joe Mills acting as project officers. They have already reached out to connect with program directors individually. They are excited to meet you all and begin connecting.
