REQUIRED INFORMATION

Payroll Reports
Payroll reports for 2019 and 2020 year to date showing the following by employee and/or officers:
  • Gross Wages
  • Paid Vacation
  • State & local taxes (940, 941 or 944)
  • Paid time
  • Pay for family medical leave
  • 1099's for independent contractors

2019 Tax Information
Completed 2019 tax return or 2019 Profit and Lost Report and Balance Sheet .

Other Documentation
  • Funds received in the form of an Economic Injury Loan since 1/31/2020
  • Payments for group health benefits, including premiums paid in 2019-2020 year to date
  • Payments of any retirement benefits paid in 2019-2020 year to date

Quarterly IRS Forms
Forms 940, 941 or 944 for these dates:
  • March 31, 2019 and June 30, 2019
  • March 31, 2020 and June 30, 2020

Payment Documentation
Documentation in the form of cancelled checks, payments receipts and bank statements showing payment of the following items form 2/15/20 - 6/30/20:
  • Mortgage interest
  • Rent payments
  • Utilities

Make an appointment, they can begin the process via phone or video and develop a process for you to safely hand of the documents. They are learning about this process as quickly as possible in order to assist you.

Rent & Mortgage Assistance
President Trump directed the Department of Housing and Urban Development (HUD) to suspend evictions and foreclosures through April as Americans grapple with the fallout of the coronavirus.
Paid Sick Leave, Prescriptions & Insurance
Emergency Paid Sick Leave - Any employee who works for an employer is eligible.
  • Full-time Employees – 80 hours of paid sick leave
  • Part-time Employees – Paid based on the average number of hours the employee worked for the six months prior to taking this paid sick leave
GA Insurance Commissioner John King instructed insurers not to cancel health insurance policies of those who miss payments until further notice.  https://www.oci.ga.gov/home.aspx
Prescriptions (Rx) can be renewed for 90 days and obtain early refills.
Drivers License & Tag Renewals
