Short Summer Childcare Survey
We are trying to determine if there is still a need for our summer BASE program.

We are requesting that all families complete this short survey in order for us to have a better understanding of the needs of our families.

We would appreciate any feedback you have on what we can do better, or why your family chooses another child care program.

Thank you!
