SIM Extension Service to Advance Integration Efforts
The Colorado Health Institute (CHI) has been selected by the Colorado State Innovation Model (SIM) Office to lead the creation of its extension service, a program designed to connect medical practices with local resources such as public health agencies across Colorado so that patients get a full range of care and support. The four-year contract calls for CHI to establish the SIM Extension Service and provide its governance, coordination and oversight.

Register for SIM Practice Facilitator and CHITA Training
The University of Colorado
Anschutz will be hosting a training event for the SIM Practice Transformation Organizations (PTOs) on January 20 and 21, 2016.
The training will focus on practice transformation as it relates to integrating behavioral health into primary care.
All SIM PTOs are encouraged to send all practice facilitators and CHITAs anticipated to work with SIM practices to the
event. This innovative training will provide a comprehensive framework outlining project expectations, overview of assessments, administration of tools, as well as detailed implementation information and panel discussions.

Save the Date for the Colorado Telehealth Consensus Conference
The Colorado Telehealth Working Group invites you to register for the 2016 Colorado Telehealth Consensus Conference on February 1, 2016. The conference
will be a one day strategic planning forum for telehealth going forward in Colorado.
The focus of the conference will be centered on the following two use cases:
- Using telehealth in clinical settings
- What will telehealth delivery look like in the future?
The primary goal for this all-day session will be the creation of a white paper outlining the strategic vision for a coordinated approach to telehealth implementation and delivery in Colorado. Your input will be critical to achieving this goal!

The Accountable Health Communities Model is currently accepting applications through March 31, 2016. The Model addresses a critical gap between clinical care and community services in the current health care delivery system by testing a systematic approach to identifying and addressing health-related social needs of Medicare & Medicaid beneficiaries.
