Updates to Application and Renewal Questions

The board recently voted to make several changes to the questions on its initial and renewal applications. These changes will take effect for all license types before the end of the year.

Some of the changes include:

  • Streamlined questions
  • Greater consistency between each application type
  • New questions that prompt that applicant to provide additional information if they provide an affirmative response
  • A question about the percentage of practice that is telehealth

Physicians (MD, DO, DPM) and Physician Assistants

In addition to the changes described above, the application and renewal application for physicians and physician assistants will include a question that asks about any supervision agreements or standard care arrangements in place. If you respond to the question with an affirmative answer and do not work in a hospital setting, you will also be required to upload a list of providers and information about their practice location. NOTE: There is an exception for agreements within a hospital setting.
