MEDICAL WAIVER & OPT-OUT CREDIT BENEFIT: If you are waiving medical coverage and/or enrolling in the Opt-Out, the appropriate forms can be obtained from your local HR Department. You will need to complete these forms each plan year to receive the Opt-Out credit and to confirm your intention to waive coverage.
FLEXIBLE SPENDING ACCOUNT (FSA), DEPENDENT CARE (FSA), AND MEDICAL OPT-OUT CREDIT MUST BE RE-ELECTED EACH YEAR.

THE MEDICAL DENTAL, VISION, AND VOLUNTARY PLANS WILL ROLL OVER AUTOMATICALLY THIS YEAR.
DEPENDENT VERIFICATION AUDIT (DVA): This year, Prime Healthcare will be verifying all existing enrolled dependents or any dependent who will be added for the 2022 plan year. You must present the appropriate documents to a Benefits Counselor to verify your dependents – see the Dependent Verification section.
