The Crisis Response Network (CRN) has recently revised several forms utilized for submission of information as part of the Seriously Mentally Ill (SMI) eligibility determination and decertification process. To ensure consistency in capturing required information, providers are encouraged to utilize these forms available via the
CRN webpage.
What's changed?
New language informing members that a determination of SMI eligibility for members already enrolled in an AHCCCS health plan will align the member with the RBHA for integrated physical and behavioral health services and may lead to changes to their existing network of healthcare providers.
CRN forms have been revised to better align with AHCCCS policies and system requirements.
Changes to the SMI decertification form and associated policy.
