Community Health Resources

Looking for support for you? A friend? A family member? Help is available.

Community Health Workers (CHWs)

A CHW is a trusted member of the community who has a strong understanding of local resources. If you are in need, a CHW can assist you with social services and navigating the healthcare system. At Trinity Health, we understand that concerns like your rent or mortgage, the price of food, reliable transportation, and other worries, along with rising prices, can have a significant impact on your health and quality of life.

When you first meet with a CHW, they will work with you to understand what you need and partner with you to find free or reduced-cost resources. The CHW can:

• Link you to community services and resources

• Help you find medical care for your needs (primary, dental, specialty, mental health, etc.)

• Help you manage your health conditions and prescriptions

To connect with a CHW, please email CHW4U@trinity-health.org

This program is free and completely confidential for patients and community members.
