Medication Reminder
To help keep all students safe, any medication, perscription or over-the-counter, that needs to be administered during the school day must have a completed Medication Authorization Form (CLICK HERE). This form must be signed by the doctor and you.
All prescription medications must be brought to the school office by a parent or guardian in their original pharmacy-labeled container with the prescription label intact.
Over-the-counter medications must also be in their original, unopened packaging and require a signed authorization form.
Students are not permitted to carry medication (including over-the-counter medications) unless specifically authorized by the school according to school policy.
No medication will be administered by school staff or the school nurse without the required signed authorization form.
