There are four required forms for all students attending VCHS. Please follow the guidelines below to ensure your student(s) have the correct paperwork on file on or before August 9th.

Optional Form #5: (Junior & Senior Students only) Lunch Permission Form required if your junior or senior student plans to leave campus for lunch.

ADDITIONAL IMPORTANT INFORMATION:

All incoming 9th graders and new transfer students are required to have a current immunization record on file on or before the first day of school (August 16th). Please email the document to Kristy Maslan at kmaslan@vchsweb.org or drop it off at the school office at your convenience.

All students participating in sports for the '21-'22 school year are required to have a physical form on file with the Athletic Director on or before the first day of practice. Students will not be allowed to participate without a physical form signed by a medical doctor. Sports physicals for VCHS athletes are available on July 22nd and August 5th at VCHS Gymnasium Building from 3-5 pm. A $15 donation is requested. You may also schedule a physical with your family physician and use the physical form provided.

VCHS Student Success Center

Sandra Burner will be heading up our Student Success Center again this year. Click here for all the information about VCHS Student Success Center. Contact Sandra Burner for more information at sburner@vchsweb.org.

Vail-Summit Orthopedics is offering athlete's sports physicals on VCHS Campus this Thursday 7.22.21 and 8.5.21 from 3:00 pm - 5:00 pm in the East Gym Building. An appointment is not needed for a physical.

Happy Healthy Brain Foundation will be offering Baseline Brain Scans by appointment only simultaneous to the sports physicals. Please click here for more information and to register. Contact Laura Nowicki at 708.366.8663 with any questions.

The Learning Center

The Learning Center after school enrichment program will begin on Tuesday, August 31st and will meet every Tuesday and Thursday that VCHS is in session from 3:15 - 5:00 pm. We welcome students from all schools in grades K-12. Homework Support is available in all subjects and extra enrichment for math and language arts is always available.
