Learn your numbers for cholesterol, blood sugar, blood pressure, and BMI, and discuss your results with a trained health coach at your free and confidential screening appointment! Complete your Health Risk Assessment prior to your appointment to review a full report of your results and set goals.

To schedule your appointment, visit the 'Onsite Biometric Screenings' section in the middle column of your Wellness Portal!

Remember: your screening is completely confidential! Your personal health information will NEVER be shared with your employer or the Trust.

During your 20-minute appointment, you will:

• Meet with a Health Coach who will take your vitals and perform a quick finger-stick to check your cholesterol and blood sugar
• Preview a downloadable report that will include your results and next steps as recommended by your screener
• Learn how to schedule a health coaching appointment on your Wellness Portal, and ask any questions you have about the program

Unable to attend an on-site screening event? You have two alternative options to complete your biometric screening:

1. Download the Provider Form on your Wellness Portal. Bring the form to your doctor to fill out your results and upload it on your portal.
2. Order a Home-Test Kit to complete at your home. Complete and return your kit and your results will be available on your portal.

Registering is easy! Simply follow these steps:

1. Visit benefitstrust.wellnessworkdays.com
2. Read the Terms and Conditions, then click ACCEPT
3. Click REGISTER, then click ACCEPT
4. Enter BenefitsTrust in the Company box (one word)
5. Enter your 12-digit Member ID (last 4 digits of SS, followed by your date of birth, written as MMDDYYYY)
6. Enter your date of birth and email
7. Click REGISTER
8. Create a password, then log in!
