Start of School Forms and Information Filling out those school forms may seem daunting, but it's important for families to submit the back-to-school packets. All families are especially encouraged to submit the Student Data Verification Form, Family Educational Rights and Privacy Act (FERPA) form and the Student Meal Assistance forms.
