A Note from the School Nurse
Starting this week if I see a student in my office I am going to be filling out a form that I have created and sending these forms home with the student for parent(s)/guardian(s) viewing.
The form explains why the student was seen in my office and if there needs to be any medical follow-up such as a COVID test.
I think these notes will help when determining what steps need to be taken for a child to return to school if seen for a COVID related sign or symptom(s).
I will also call and speak to the parent(s)/guardian(s), but I think having something written on paper will help assist and decrease any confusion.
I have attached the form for viewing.
Below is the full list of symptoms for which caregivers should monitor their children, and staff should monitor themselves:
❏ Fever (100.0° Fahrenheit or higher), chills, or shaking chills (CDC has lowered the temperature from 100.4 to 100.0)
❏ Cough (not due to other known cause, such as chronic cough)
❏ Difficulty breathing or shortness of breath
❏ New loss of taste or smell
❏ Sore throat
❏ Headache when in combination with other symptoms
❏ Muscle aches or body aches
❏ Nausea, vomiting, or diarrhea
❏ Fatigue, when in combination with other symptoms
❏ Nasal congestion or runny nose (not due to other known causes, such as allergies) when in combination with other symptoms
