Please note that our office will be closed Monday, December 26th and will reopen Tuesday, January 3rd. CCMS Staff will monitor email and voicemail and will respond to urgent messages as soon as possible. Deliveries can be dropped off at the Neighborhood Health Clinic front desk, located in the same building as CCMS (note, the clinic is closed 12/23, 12/26 and 1/2).
