Estimados Padres de Familia de Secundaria:
 
Nos complace informarles que la IPS Red Ladera, ha confirmado la realización de la segunda jornada de vacunación con PFIZER para el próximo viernes, 5 de noviembre para nuestros estudiantes mayores de 12 años. En esta jornada se estará administrando primeras y segundas dosis de la vacuna.
 
La jornada está programada para iniciar a partir de las 9:00 a.m. con el personal de la IPS y el acompañamiento del Equipo Médico del Colegio Bolívar.
 
Si desean que su hijo(a) reciba la vacuna durante esta jornada, les solicitamos cumplir con los siguientes requerimientos:

Requerimientos Primera Dosis:

  1. Enviar el Consentimiento Informado para la aplicación de la vacuna, emitido por el Ministerio de Salud y Protección Social, IMPRESO y debidamente diligenciado con el estudiante. El padre de familia o acudiente debe firmar el consentimiento como representante legal y el estudiante debe firmar como la persona que recibe la vacuna. Además, se les solicita agregar en este documento el correo electrónico del estudiante y del acudiente quien firme.
  2. Diligenciar el Cuestionario Condiciones de Salud del estudiante en la plataforma PowerSchool antes de abordar el bus o ingresar al Colegio ese día, ratificando que no presenta síntomas asociados con COVID-19.
  3. El estudiante debe portar su Tarjeta de Identidad o Cédula de Ciudadanía original y traer fotocopia de dicho documento.

Requerimientos Segunda Dosis:

  1. El estudiante debe cumplir con todos los requisitos descritos para la primera dosis: Consentimiento Informado y el diligenciamiento del Cuestionario de Salud en la Plataforma PowerSchool. Además, debe portar el Carné de Vacunación físico con el registro de la primera dosis y la fotocopia de su Tarjeta de Identidad o Cédula de Ciudadanía.
  2. El estudiante que cuente con la primera dosis podrá recibir su segunda dosis siempre y cuando se haya cumplido con el mínimo de 21 días de haberse vacunado.
 
Todos los requisitos anteriormente mencionados deben cumplirse con el fin de recibir la vacuna.
 
No duden en comunicarse con el Servicio Médico del Colegio en caso de tener alguna inquietud adicional.

Dear Secondary Parents,

We are pleased to inform you that the IPS Red Ladera, will be at Colegio Bolívar this Friday, November 5, offering the second dose of PFIZER vaccine for our students over 12 years of age. On this day, the first and second doses of the vaccine will be administered.

We are scheduled to begin at approximately 9:00 a.m. with IPS Red Ladera personnel accompanied by Colegio Bolivar’s Medical Team.

If you wish your child to receive the vaccine on that day, you must comply with the following procedures:

First Dose Requirements:

  1. Send with the student a PRINTED and duly completed Consent Form issued by the Ministry of Health, for the application of the vaccine. The student must sign the consent as the person receiving the vaccine and the parent or guardian must sign as the legal representative. It is necessary to include the student’s and parent’s emails.
  2. It is also necessary to complete the Health Conditions Questionnaire of the student on the PowerSchool platform before entering the school or boarding the bus that day, confirming that there are no symptoms associated with COVID-19.
  3. The student must have their original and photocopy of their Identity Card or Cedula with them to receive the vaccine.

Second Dose Requirements:

  1. The student must comply all requirements described for the first dose: Consent Form and the completion of the Health Conditions Questionnaire on the PowerSchool platform. As well as, the Vaccination Card with the registration of the first dose and the photocopy of their Identity Card or ("Cédula").
  2. The student who has the first dose may receive his second dose as long as the minimum of 21 days after being vaccinated has been met.

All the above requirements must be met to receive the vaccine.
