This month we highlight a new sfCare resource; a free training program for primary care; a webinar on alcohol misuse, and 10 things anyone can do right now to make the care they provide more senior friendly.
NEW sfCare Tools!

New resource added to the sfCare Getting Started Toolkit :

This slide deck includes speaking notes, and will help you familiarize  senior friendly team members (including champions, steering committees, working groups etc.) with the sfCare Framework, and all that the sfCare Getting Started Toolkit has to offer, such as an online self assessment tool with report card, implementation resources, and training modules for executives and staff. 

New resources launching late Sept 2018:
  • The SF7 Toolkit will promote integrated care across sectors by providing information for healthcare providers and older adults /caregivers on: cognition/delirium, mobility/falls, continence, nutrition, pain, polypharmacy, and social engagement.
  • The sfCare Forum will promote a collaborative community where healthcare providers can learn from and inspire each other by sharing experiences and tools, asking others for advice, or reading a topic of interest.
Free Care of the Elderly Educational Program for Primary Care

Baycrest Health Sciences, in partnership with the North East Specialized Geriatric Centre, is launching the second cycle of " Project ECHO Care of the Elderly" (ECHO COE) - a telehealth program that aims to help primary care providers build capacity in the care of older adults through weekly video conference sessions.

ECHO COE will be held weekly on Wednesdays from 3:30-5:00 pm, from Sep 5, 2018 to Dec 19, 2018, with the exception of Sep 19, 2018.

The goal of this free educational program is to enhance the quality of care for older adults in Ontario, by improving the knowledge and skills of primary care providers. ECHO COE will connect primary care providers to an interprofessional team of geriatric experts through weekly videoconferencing sessions, which will include didactic lectures and case-based learning. CME credits will be provided.
No special equipment is required - just a computer, tablet or smartphone with internet access.

Introduction to the Senior Alcohol Misuse Indicator (SAMI) Tool , by Dr. Bonnie Purcell, Ph.D., C.Psych.

About the webinar:
This webinar will explore the changes that occur with aging that necessitate a different approach to screening for alcohol use with older adults; and provide an introduction to  SAMI, a screening tool that provides a gentle, non-confrontational approach to elicit information to determine if there are concerns related to alcohol use/misuse.

About the presenter:
Dr. Purcell is a registered clinical psychologist with the London-Middlesex Behavioural Response Team and Geriatric Mental Health Program with London Health Sciences Centre. She has been working in the field of alcohol-related problems among the senior population for 15 years.
