  • HCPC 21.1 - Patient’s individualized written plan of care includes planned interventions based on problems identified in the initial and updated comprehensive assessments.


  • HCDT 16.1- Hospice aide provides services ordered by the IDG and included in the plan of care.


  • HCPC 15.1- The comprehensive assessment includes a drug profile that contains the patient’s current prescription and over-the-counter (OTC) drugs with medication regimen review process.
