The Student Affairs Office is responsible for student clubs, activities, leadership programs, assemblies, pep rallies, dances, special events and celebrations, transportation and the daily bell schedule.
Please visit the School Nurse's website for details about the required 11th grade physical, Medication Policies and Procedures, and immunization requirements. Check the back of documents to make sure everything is complete!
Section 7 must be uploaded onto your familyid account.
This must be done by the first day of the practice. No athlete will be permitted to practice/ tryout with the proper physical forms and registration.
Transfers
If you are a transfer student, please email BJ Hogan. O'Hara will need to get your former school to sign off on the PIAA transfer request.
Information about your Doctor’s PIAA Physical Section 7 Form:
Every athlete will be required to have a doctor’s physical, which needs to take place AFTER June 1, 2021. Physicals dated before June 1, 2021 are not valid.
The doctor performing your physical will need to fill out Section 7 of the PIAA physical paperwork. You will need to print that form and bring it to the doctor on the date of your physical so that he/she can sign it once the physical is complete. Once complete and signed, this is what you will scan and upload to Family ID.
Important: Only the official PIAA Section 7 form will be accepted for registration.
Athletes who upload physicals on a different form other than the Section 7 form will not be approved to participate.
