  • Reminder to fill out all parts of Page 1 of the Assessment.
  • Anything that is not applicable to your patient just write N/A.
  • Please make sure the caregiver dates after signing on page 1 and 2 of Assessment.
  • You may fill in the date for them prior to leaving the home for that shift.
  • When you receive the monthly MARs make sure to review with POT with any new orders since POT printed.
  • Then please sign reviewed by in the top left corner of each page of the MAR.
  • Also please remember to sign the bottom of each MAR page with credentials, even if medications on the page are not given by you.
  • Make sure to add a blank Addendum to the POT form with the Emergency/To Go Bag.
  • If the nurse accompanies the patient to MD visit please have either the Addendum filled out or an MD visit form listing the medications.
  • The nurse in the home can give medications, new orders or changes in orders if listed on the MD Visit paperwork, Hospital Discharge paperwork, or a bottle of medication in the home.
  • Just fill out an Addendum for the medication bottle in the home.(That way the office can obtain a signed order for the medication.)

Thank you to our Nursing Supervisors for continuing to improve and update our POTs.
Field Nurses please communicate any changes to POT promptly to the Nursing Supervisors.
This includes monthly weights and vaccinations from patient MD appointments.

Controlled Narcotics Documentation
Controlled Narcotics need to be counted at the beginning and ending of the shift with either another nurse or a caregiver in the home. Both parties will need to sign off on the Controlled Substances Log.  At the time of counting not on another day!

Reminder to fill out the bottom of each MAR page to show you have reviewed medications given to patient. Even if there are no medications on that page that the nurse has given.

* Just a reminder - New orders or discontinued orders (D/C) must be documented in the following manner:
  • Fill out an Addendum to the plan of treatment.
  • Add the order to the MAR or TAR.
  • When filling out the MAR or TAR you must include the date of the order, initial under the date.
  • If the order is being D/C indicate by a single line through the order and date and initial.
  • Any new order or D/C order will also need to go on the POT, in the communication log, on the medication profile, narrative note, yellow copy of Addendum placed in chart and you must call the Supervisor for the case.
  • Medications must be written in mg and in parenthesis you may add the (ml) amount.
  • The reason for this is concentrations change.
  • In addition all pages of the MAR are reviewed and signed for top left corner prior to the start of every month

September 9-15 is Neonatal Nurses Week-Celebrating You!

Neonatal Nurses Week-Celebrating You!
This year NANN is expanding Neonatal Nurses Day to an entire week, September 9-15! Established in 2000 by the National Association of Neonatal Nurses (NANN), members of the neonatal community use this time to honor their nurse colleagues and show their pride in being a neonatal nurse.
Neonatal Nurses Week provides an opportunity to celebrate the hard work and dedication of neonatal nurses as well as the NANN community. Your contributions to the tiniest patients make a difference that lasts a lifetime for neonates and their families.

Neonatal Nurses Week - We Save Babies!
The Neonatal Nurses Week theme had a very unexpected origin. It started in late 2016 at NANN's 32nd Annual Conference in Palm Springs when attendees were connecting to the conference Wi-Fi on their many devices. The password to connect was "WeSaveBabies" and our attendees instantly loved it! Who knew a simple password could have such an impact on our attendees? Nurses continued to reference the phrase throughout conference and it took on a life of its own. We knew it was perfect for Neonatal Nurses Week!

September is Baby Safety Month

September is Baby Safety Month, sponsored annually by the Juvenile Products Manufacturers Association (JPMA). This month-long event increases consumer awareness of safety issues and the safe selection and use of baby products. Whether you are a retailer, manufacturer, industry partner, doctor's office, or playgroup, you are in a wonderful position to spread the word to parents and caregivers about baby safety throughout Baby Safety Month and all year long.

This year, JPMA is helping educate parents and caregivers on the importance of properly choosing and using all the best products for your child's age and developmental stage as outlined by the manufacturer.

September is National Cholesterol Education Month

Children, young adults and older Americans can have high cholesterol. Learn how to prevent high cholesterol and know what your cholesterol levels mean.

September is National Cholesterol Education Month, a good time to get your blood cholesterol checked and take steps to lower it if it is high. National Cholesterol Education Month is also a good time to learn about lipid profiles and about food and lifestyle choices that help you reach personal cholesterol goals. Below you will find some information about cholesterol and a summary of CDC programs that address cholesterol across the country. You will also find a few fact sheets and publications about cholesterol, as well as links to useful consumer and health care provider information on our partner Web sites.

September is Food Safety Month
The Core Four Practices

Right now, there may be an invisible enemy ready to strike. He's called BAC (bacteria) and he can make people sick. In fact, even though consumers can't see BAC - or smell him, or feel him - he and millions more like him may already be invading food products, kitchen surfaces, knives and other utensils. But you have the power to Fight BAC!® and to reduce your risk of foodborne illness.  It's as easy as following these core four Fight BAC!®practices for food safety:

Best NJ Labor Day Weekend Events
2019 concerts, festivals, parades, fireworks, and more

Labor Day, the first Monday in September, falls on September 2, 2019 this year.

Created as a labor movement dedicated to the social and economic achievements of American workers, It constitutes a yearly national tribute to the contributions workers have made to the strength, prosperity, and well-being of our country.

Over the years this holiday has evolved from a being a labor union celebration into a celebration of the unofficial end of summer, especially for families with kids.

Labor Day is often regarded simply as a day of rest and or end of summer celebrations that include picnics, barbecues, fireworks, parades, water sports, and public art events. Families with school-age children take it as the last chance to travel before kids begin the new school year.

Loaded with zucchini and carrots, these tots do not even taste healthy! It's the perfect way to sneak in veggies, and it's just so good!
2 zucchini, shredded
1 teaspoon salt
2 carrots, peeled and shredded
1/2 cup Panko*
2 large eggs
1/4 cup shredded cheddar cheese
2 tablespoons freshly grated Parmesan
1/2 teaspoon dried basil
1/2 teaspoon dried oregano
1/4 teaspoon garlic powder
1/4 teaspoon onion powder
Pinch of cajun seasoning
Kosher salt and freshly ground black pepper, to taste
2 tablespoons chopped fresh parsley leaves

  1. Preheat oven to 400 degrees F. Lightly oil a baking sheet or coat with nonstick spray.
  2. Place grated zucchini in a colander over the sink. Add salt and gently toss to combine; let sit for 10 minutes. Using a clean dish towel or cheese cloth, drain zucchini completely, removing as much water as possible.
  3. Transfer zucchini to a large bowl. Stir in carrots, Panko, eggs, cheeses, basil, oregano, garlic powder, onion powder and cajun seasoning; season with salt and pepper, to taste.
  4. Form zucchini into tots and place onto the prepared baking sheet.
  5. Place into oven and bake until golden brown and crisp, about 16-18 minutes, flipping during the last 10 minutes of cooking time.
  6. Serve immediately, garnished with parsley, if desired.*
*Panko is a Japanese-style breadcrumb and can be found in the Asian section of your local grocery store.
*TO FREEZE: Transfer cooled tater tots to an airtight container. Freeze up to 1 month. To bake, preheat oven to 400 degrees F and bake until heated through, about 15-17 minutes.
