Compliance deadline is October 25.
Non-compliant students face academic holds.
All Fall 2019 incoming students who do not comply with the UC TB/Immunization mandate by the deadline date of October 25 will have an academic hold placed on their record preventing enrollment in Winter quarter (or Spring semester) classes.
Non-compliant students will receive secure messages from Student Health Center advising them of their compliance status. New, incoming students should immediately take the following actions if not already completed:
- Log on to the Student Health Patient Portal.
- On the homepage, select Medical Clearances from the menu:
- Enter the dates by clicking "update" for each vaccination that you have already received.
(Note: Please complete the Medical Clearances even if you have not completed ALL immunizations.)
- Complete the TB Risk Screening Questionnaire Form.
- Upload your Immunization and TB records.
(Note: You may upload additional documents at a later date, as you obtain them, by clicking on "update" next to the Immunization Records section.)
Special immunization clinics will be held at the Student Health Center starting the week of September 23 through October 25 to accommodate any students who still require immunizations and/or TB testing following the start of the fall quarter. Students may call 949-824-5304 to schedule an appointment.
After the deadline and after academic holds are placed for Winter Quarter, non-compliant students will still have an opportunity to complete the requirements and have the academic holds removed. However, the delay could adversely impact class availability and enrollment.
