The Community Health Worker (CHW) Forum was led by the CDC, National Center for Chronic Disease Prevention and Health Promotion’s Division of Diabetes Translation to gather CHWs, CHW allies, and state health department representatives to explore issues related to developing a statewide infrastructure to promote long-term sustainability and financing of CHWs.
