In our efforts to continually enhance the CPP rebate program, we are now sending rebates twice per year! Members that participate in the vaccine buying group should have received the first rebate check of 2021 in April, which reflects vaccine purchases between July 1 – December 31, 2020. The next rebate check will be sent in the Fall of 2021, reflecting vaccine purchases between January 1, 2021 – June 30, 2021. We hope this is beneficial as you recover from the pandemic.

and return to CPP by email at CPP@Nationwidechildrens.org
or FAX to (614) 722-2149. Once received, we will take care of the rest. CPP will notify you when your earned rebate funds have deposited into your account. If you have already submitted an ACH form to CPP, you do not need to complete it again.
