Join us to learn about the Medicare GUIDE Model, a new program designed to support both individuals living with dementia and the family members who care for them. The GUIDE model offers a no-cost dementia care coordination, education, emotional support, and respite services, giving caregivers relief, guidance, and someone in their corner and those they love and care for with dementia a more coordinated plan to navigate the journey of dementia care they are on.
During this session, we’ll walk through:
- How the GUIDE model supports caregivers’ day to day
- What services are available and who may be eligible
- How to apply and what the enrollment process looks like
- How to get help sooner, without added stress or paperwork overwhelm
