What Referrals/Resources Did a Family Receive?

Each month, Help Me Grow shares tips with Sparkler Program Partners to help them get the most from the Sparkler dashboard. This month, we want to focus on helping you know what referrals or resources were provided to a family.


Help Me Grow Developmental Screening Specialists offer these referrals and/or resources when they contact the family to share the child's ASQ results.

Individual Child Referral

Each screening has its own Follow-up Form that details when and how Help Me Grow contacted the family and any referrals discussed or provided.


To see this information, log onto your Sparkler Dashboard from a desktop or laptop computer.


Navigate to the ASQ Dashboard tab.The last column on the tab is titled Follow-Up.


Group of Children Referrals

You can generate a CSV (comma-separated values) Report that details all the items found in the individual Follow-Up reports.


These reports can be filtered by:

  • ASQ type
  • Date
  • Cohort

When viewing the CSV report, column U <The following referrals were made. Check all that apply.> will give you information about the referrals shared with families. You can further sort this report by types of referrals (i.e. Family accepted Early Intervention (EI) referral).  


Once the report is done generating, it will appear in your downloads. Then you can view, save, and/or print the report.


Program

You can also view referral data for your entire program on the dashboard's homepage. Scroll down towards the bottom of the page and look for the Follow-Up Recommendations pie charts. If you hover over a slice of the pie, you will be able to see how many children were referred to Early Intervention (EI), Home Visiting (HV) and Preschool Special Education. 

**Reminder**

We can only offer referrals/resources if we are able to contact the family. You can help us by encouraging your caregivers to respond to the in-app message (see image below) they receive as soon as they hit submit after completing a screening.

If a family does not want to be called by Help Me Grow, they can also respond to this automatic message and ask us to email them their child's results. 


If you have concerns about a child, regardless of what the screening scores indicate, you are encouraged to discuss your concerns with the child's caregiver and then submit a referral to Help Me Grow.

Help Me Grow is a statewide system of supports that promotes the healthy growth and development of young children from pregnancy through early childhood. Services and supports are provided statewide through Home Visiting, Early Intervention, developmental screenings, and connections to community resources.

The newest service offered through Help Me Grow is the universal newborn home visiting program, Family Connects Ohio.

Any family in a participating community, regardless of income, can sign up for a no-cost visit from a registered nurse about three weeks after baby's arrival.
