PERSON CENTERED PLANNING

TRANSITION TO ADULTHOOD


Person-Centered Panning is a collaborative and respectful process that places the student at the heart of al decision-making. Through facilitated conversations ad inclusive methods the planning team works to gain a deep, shared understanding of the student's strengths, goals, and needs. The student remains actively involved throughout the process, which is conducted in a welcoming equitable and supportive environment that encourages the voices of all participants.
