Yamhill Community Care (YCCO) would like to share the following important information and updates for Providers:

New Updates & Reminders!

YCCO Provider Handbook has been updated!

Available 8/1/2025


The Yamhill CCO Provider handbook is a resource that contains information on your responsibilities as a Yamhill CCO network Provider, health plan benefit information, and required policies and procedures.


Please pay special attention to sections 9.7 and 9.8 for updates to the grievance and appeal process.


Care Coordination & Community Health Hub



As a provider, you can support your patient by requesting a Care Coordinator, Care Manager or a Community Health Worker. Our Community Health Hub is equipped to address the needs of your patients and connect them with valuable resources.

Referrals can be made by anyone in your office, local community partners, or through healthcare professionals involved in the patient’s care.

To request a Care Manager or a Community Health Worker for your patient or their eligible family members, simply contact Customer Service at 1-855-722-8205 or fill out the form.

Care Management and Community Health Hub Request

Once the referral is made, your patient will receive personalized, one-on-one support tailored to their specific needs.

Important billing update


Effective 7/1/2025, YCCO will pay standard DMAP rates for any provider who has not completed their YCCO credentialing from June 1, 2024 to April 1, 2025. 


 Behavioral Health Prior Authorization Changes

 

Currently YCCO has an administrative PA referral via the CIM system requirement for payment of outpatient behavioral health services, this process will be removed beginning 8/1/2025 and no longer be required to be paid for the previously indicated services.  

 

The BH Services that require prior authorization have been added to the YCCO PA List. The YCCO PA List is located on the website here  https://yamhillcco.org/providers/policies-and-forms/ as well as via CIM in the Provider Services section.  

 

The BH Grid currently on the YCCO website will be retired on 08/1/2025 and will be removed. 


Family Core


Are you working with someone who is pregnant or has a child under 5 that lives in Yamhill County? Do they need parenting support, early childhood education, health resources, or birth and post-partum support? Fill out a FamilyCore referral at: https://yamhillfamilycore.org/request-services/. Someone will get in touch with them shortly!

Recent rule revisions

OARs 410-125-0141, 410-125-0195, 410-120-1295: Rescinds the wage index rule changes enacted July 1, 2025, restoring the rules to their prior state before that effective date. OHA will immediately start the permanent rulemaking process; this will allow for further community engagement in the rulemaking process.

OAR 410-121-0040: Updates the Oregon Medicaid Pharmaceutical Services Prior Authorization Criteria, implementing recommendations from the Pharmacy and Therapeutics Committee.

YCCO invites you to bring your questions and concerns about language access service and data collection, particularly as it relates to the OHA metric and the data requests you are receiving from us.
