I am so excited about your decision to begin supervision. Below is a list of instructions for how we can get started. Please notify me if you have any questions or concerns regarding this contract.
The Supervision Plan is the main document we need to complete. The state will retroact all contracts, which allows us to begin supervision at our discretion. The instructions below will support you with completing this contract and making your first payment.
Step #1: Complete The Supervision Plan
Please complete sections 1, 3, 4, and 5. In section 3, document the practice location of your employer. Please remember to sign and date the document in section 5. Please do not sign with an electronic signature.

Where Do I Send The Supervision Plan?
Once you have completed the Supervision Plan you can forward it to me at choya_w@aspirecounselingal.com. I will complete the forms and mail them to the state.

Step #4: Schedule a Follow-Up Meeting on Zoom
We will need to meet again briefly to discuss our supervision goals via Zoom. Please schedule a follow-up meeting via Zoom. This meeting will take place after the completion of all supervision documents.
