Dear Chartiers Valley families,
If you have a student entering 3rd, 6th, 7th, 11th, or 12th grades in the fall of 2021, please be aware of the following Pennsylvania Department of Health guidelines regarding physical exams and vaccinations.
- 3rd Grade

A dental exam is required, preferably by your family dentist. Any dental exam performed after August 20, 2020 will be accepted. Have your dentist complete this dental form.
- 6th Grade

A physical exam is required, preferably by your family doctor. Any physical exam performed after August 20, 2020 will be accepted. Please have your doctor complete this physical form.
- 7th Grade

A dental exam is required, preferably by your family dentist. Any dental exam performed after August 20, 2020 will be accepted. Have your dentist complete this dental form.
- One dose of tetanus, diphtheria, acellular pertussis (Tdap), and one dose of meningococcal conjugate (MCV) is required to begin 7th grade unless there is a documented medical or religious/philosophical exemption.
- 11th Grade

A physical exam is required, preferably by your family doctor. Any physical exam performed after August 20, 2020 will be accepted. Please have your doctor complete this physical form.
- 12th Grade
- One dose of meningococcal conjugate (MCV) is required to begin 12th grade unless there is a documented medical or religious/philosophical exemption.
