Future Planning: One Step at a Time
home_keys_planning.jpg
Taking the first step towards future planning is important.  And it's not quite as scary as you may think.

The first step is to gather some basic information.  You have now completed the About Me, Where I Live, and Daily Activities and Social Connections worksheets.  It's time to take a look at Daily and Major Life Changes.  In the Daily and Major Life Changes section, you will gather information about your legal arrangements pertaining to a guardian or power of attorney.  This 2-page worksheet will help you keep important information in one place.

You can go through the process online in an interactive and user friendly format.  Or, you can start by printing a paper copy.
