As we begin 2023 following years of unprecedented challenges in Covid-19 and now switching from response mode to recovery, I just want to express my sincerest appreciation for the amazing network of providers we have both on and off island serving our TakeCare family.  Your commitment does not go unnoticed.  

We are at a critical juncture in medicine.  According to 2020 data published by the Association of Medical Colleges, the United States could see a shortage of 54,100 to 139,000 physicians by 2033.  If those statistics weren’t scary enough, imagine what healthcare in Guam would be like 10 years from now considering how extremely delicate our practice environment is already.  The medical disparities that exist locally compared to the U.S. mainland and Hawaii are significant.  Our unique circumstances, lack of resources, and remote location, make our precious delivery systems extremely vulnerable.

Primary and secondary prevention strategies are needed now more than ever.   According to the NCHS, heart disease continues to kill nearly 700,000 Americans each year, cancer over 600,000.  Diabetes, lung, liver, and kidney disease still high up there.  In Guam, these numbers are even more exaggerated and the effects more evident.   The American Cancer Society estimates that in 2021 roughly 1.1 million less women were screened for breast cancer and 700,000 fewer men screened for prostate cancer compared to 2019.  

We have the power to change this!  Reach out to your patients, today, tomorrow, and the next day, and the next.  Be relentless in your pursuit until every single person has had their annual physical.  Until every single person is current on breast, cervical, lung, and colorectal cancer screenings.  Until every person with mental health issues has been heard.  Until every child and family is educated and aware.  
As medical professionals we have the honor and the privilege to care for people every single day.  We have the opportunity to save lives.

Get screened for Colorectal Cancer

March is Colorectal Cancer Awareness Month, and a good time to learn more about colorectal cancer (cancer of the colon and rectum) and how it can be prevented or best treated.

Colorectal cancer  is the third most common cancer to affect Guamanians, and is the second leading cause of cancer-related  deaths for both men and women combined in Guam.

That said, colorectal cancer is a disease that can be prevented through regular screenings, a healthy diet and regular exercise. Regular screening also allows for early detection, which is key to effective treatment. We have a lot to improve on though, as overall, only 53.9% of all persons aged 50 and above in Guam have met U.S. Preventive Services Task Force screening standards compared to 74.3% in the U.S.

How can I lower my risk?
To lower your risk of colorectal cancer, multiple medical societies including the the American Society of Colon and Rectal Surgeons, the American Cancer Society, and the U.S. Preventive Services Task Force recommends that you:
      Get regular colorectal cancer screenings after age 45. Between 80-90% of colorectal cancer patients are restored to normal health if their cancer is detected and treated in the earliest stages.

Other ways to lower your risk include:

      Eat a low-fat, high-fiber diet.
      If you use alcohol, drink only in moderation. If you use tobacco, quit. If you don't use tobacco, don't start. Alcohol and tobacco in combination are linked to colorectal cancer and other gastrointestinal cancers.
      Exercise for at least 20 minutes three to four days each week. Moderate exercise such as walking, gardening or climbing steps may help.

Can colorectal cancer be cured?
Since there are very few symptoms associated with colorectal cancer, regular screening is essential. Screening is beneficial for two main reasons: colorectal cancer is preventable if polyps that lead to the cancer are detected and removed, and it is curable if the cancer is detected in its early stages.
If detected, colorectal cancer requires surgery in nearly all cases for complete cure, sometimes in conjunction with radiation and chemotherapy. Between 80-90% of patients are restored to normal health if the cancer is detected and treated in the earliest stages. However, the cure rate drops to 50% or less when diagnosed in the later stages.

Who is at risk for colorectal cancer?
The risk of developing colorectal cancer increases with age. All men and women aged 45 and older are at risk for developing colorectal cancer, and should be screened. Some people are at a higher risk and should be screened at an age younger than 45, including those with a personal or family history of inflammatory bowel disease; colorectal cancer or polyps; or ovarian, endometrial or breast cancer.

Current screening methods include fecal occult blood testing (a simple chemical test that can detect hidden blood in the stool), a digital rectal exam, flexible sigmoidoscopy (a visual examination of the rectum and lower portion of the colon), a CT colonography (performed by a radiologist), and a colonoscopy (a visual examination of the entire colon, which is the gold standard). Colorectal cancer screening costs are covered by Medicare and many commercial health plans. You should find out from your surgeon or other healthcare provider which screening procedure is right for you and how often you should be screened.

Telemedicine continues after public health emergency period (PHE)

The Department of Health and Human Services (HHS) is planning for the federal Public Health Emergency (PHE) for COVID-19, declared under Section 319 of the Public Health Service (PHS) Act, to expire at the end of the day on May 11, 2023.

Refer to Fact Sheet: COVID-19 Public Health Emergency Transition Roadmap released on February 9, 2023, from HHS.

Please note that TakeCare will continue with its coverage for Telemedicine Services for ALL plans after the PHE period. This means that TakeCare Member’s cost shares such as co-pays, coinsurance, deductibles associated with telemedicine services are waived only when such services are related to COVID-19 testing and the corresponding visit/consultation. Otherwise, the member cost-share applies based on the member’s benefit covering telemedicine.

Provider satisfaction survey

Every year, we ask you to complete a survey about your experience with TakeCare. If you completed the survey, we thank you for your participation. Your opinion is important to us, and we are eager to use your feedback to help direct administrative and operational changes to our health plan and to point out areas that needs improvement.

The Provider Satisfaction survey was administered via mail and internet to qualified respondents who were active network providers in 2022. Last year, we obtained an 18.8% response rate based on a sample size of 368. This year, we continue to partner with SPH Analytics to conduct the survey later this year. We encourage all network providers to take the time to complete the survey so we can have an accurate evaluation of our services and help us determine specific areas for quality improvement.

We received an overall satisfaction rating of 53.7% with 82.1% positive response to recommending TakeCare to other physicians’ practices. We have identified the top three measures to focus on improving, particularly in the areas of Utilization and Quality Management, Health Plan Call Center Staff and Finance Management. We are committed to continuously improve the quality of service we provide and to meet your expectations.

Provider directory information

The Consolidated Appropriations Act, 2021 (CAA), enacted on December 27, 2020, Title I (No Surprises Act) and Title II (Transparency in Coverage) establishes new requirements for health plans and providers regarding provider directory information. The law requires health plans publish in their website a list of participating provides and facilities with their directory information (name, specialty, address and contact numbers). While full guidance on enforcement is forthcoming, we are exercising a good faith approach to maintain an accurate information displayed in our provider directory.

REMINDER: USPSTF A/B GUIDELINES

In preparation for March, Colorectal Cancer Screening month, please note that as of May 2021, the US Preventive Services Task Force (USPSTF), recommends screening for colorectal cancer in adults aged 45 to 49 years as grade “B” and screening for colorectal cancer in all adults aged 50 to 75 years as grade “A”.

Commercial members with the Wellness Incentive Program are calculated six (6) months after the end of the benefit period and payment will be made within thirty (30) business days.  Members under the Federal Wellness Supplemental Program or Government of Guam Medical Plans are calculated every quarter and payment will be made within thirty (30) business days. CPT E/M BILLING REMINDER

The American Medical Association (“AMA”) that holds copyright in CPT and the Center for Medicare & Medicaid Services (CMS) implemented major revisions related to office and outpatient Evaluation/Management (“E/M”) codes between 99201 to 99215. Current Procedural Terminology E/M MDM table for higher-level codes require the clinician to meet specific MDM guidelines and documented time spent on the date of the encounter. There as this position warrants clinicians to submit medical records to TakeCare for high-level codes such as 99214 and 99215.
