CARE IN ACTION

Integrated Care, at the Speed of Primary Care

Primary care teams face packed schedules, while patients with complex medical and social needs often require more support than busy clinicians can provide.

At a pediatric practice in Mastic, HEALI Community Health Worker Jenniffer Jimenez is showing what integrated care looks like in action.

By adapting her work to the practice's pace and daily routines, Jenniffer helps connect families with social care services while easing the burden on the clinical team.

INSIDE PRIMARY CARE PARTNERSHIPS

Meet Blanka Oh and Jake Ryan, the Team behind HEALI's Health Care Partnerships

Every health care practice operates differently, and bringing community health workers (CHWs) into care teams takes more than a one-size-fits-all approach.

Meet Blanka Oh and Jake Ryan, the HEALI team members working with health care partners across Long Island to make integrated care work for their practices and patients.

From building relationships to solving day-to-day workflow challenges, they help CHWs become an extension of the care team, connecting patients with the resources and support they need beyond the exam room.
