COVID-19 Vaccine Portal
If you are interested, please fill out the COVID-19 Vaccine Portal!
Healthcare Worker Vaccinations

All GW healthcare workers are eligible for vaccination and can schedule by completing the portal registration and will then be immediately directed into the appointment scheduling site. There is no longer a need to wait for an email.

How do I get the vaccine?
If you have not yet been vaccinated and would like to do so, register in the portal even if you have done so previously (https://cri-datacap.org/surveys/?s=CMCTYLCW88).

What if I'm unable to make it to my appointment?
If you are unable to make your appointment, use the link in your confirmation email to reschedule or cancel so we minimize extra vaccines at the end of each day.

What if I've changed my mind and now would like to get the vaccine?
Please re-submit to the portal. Visit the same link again and complete a new form indicating that you wish to get the vaccine. From there, you will be taken to a link to schedule your appointment.

For more information, please visit our GW vaccine website: smhs.gwu.edu/covid-19-vaccine. If you have any questions, contact: [email protected]
