REMINDERS

Summer Move?

If you will be moving over the summer, please let the Douglas Office know as soon as possible. You will need to sign a record release form so that we can send your child(ren)'s records to their new school. Please remember the Douglas School office is closed part of the summer and therefore we may not be able to send them on short notice. Please email Sharen Crooks at [email protected].

New Updates from the Health Office

1. Please plan to pick up EpiPens, inhalers, and any other medications ON OR BEFORE THE LAST DAY OF SCHOOL (JUNE 15th), DURING SCHOOL HOURS. **Please contact the School Nurse before coming to pick up any meds, and also inform the nurse if you need to make alternate arrangements for pick-up, or if your child will be attending summer school here.**

 2. Remember, all medications should preferably be delivered/picked up directly to/from the school nurse by a parent or other responsible adult. It can be unsafe to send medication to and from school in your child’s backpack, as it could end up in the wrong hands.

3. On the Acton Boxborough School Nurse Website you will find a link for “Forms” -- forms should be completed, signed by your licensed care provider, and returned to the School Nurse on or before the first day of school, August 30th, 2021. Children with life-threatening allergies, asthma, seizures, or diabetes must have a care plan completed and signed by their physician each year. Medication cannot be administered until the nurse has a signed, completed authorization form from the healthcare provider.
